Question: How Do I Turn A Paper Form Into A Web Based Fillable PDF Form?

From time to time visitors to 4xPDF send us questions about what software solution we think they should use. Going forward we’re going to start publishing these questions (private details removed of course) and let visitors to our site suggest suitable solutions. If you think your product (or you know of a product that) meets the asker’s requirements then please leave a comment (we’ll provide a summary of the best comments to the asker).

Question: I am looking for a solution to meet the following requirements. We need a solution that turns our paper based forms into fillable PDF forms and then lets us put these PDF forms on our website with a connection to our database. We would prefer one application or solution that meets all of these requirements and it needs to be part of an automated work flow. What do you suggest?
